This wonderful organisation has helpful information on how to start a conversation with your family about things they may not want to talk about with you. You can complete online or download to complete by hand an Advance Decision, sometimes called a Living Will. An advance decision form (sometimes known as an advance decision to refuse treatment, an ADRT, or a living will) lets you refuse any medical treatments that you do not want to be given in the future.
