Cremation Services
What does a Cremation Service involve?
Options for the venue locally are Wyre Forest Crematorium and Stourbridge Crematorium. With other crematoria such as Waseley Hills, Gornal Wood, Rowley Regis and Worcester slightly further away.

While all services are unique and can be adjusted to your requirements. Generally a service at the crematorium will go like this:
The hearse and any family cars will arrive about 5 - 10 minutes before the time slot for the service. This gives us a few minutes to check everything is ready and for you to greet others and prepare.
We will then take the coffin into the chapel, led by the minister or celebrant taking the service, and place it onto the catafalque.
The funeral director will be there at the end to lead you out of the chapel to the flower terrace. It is also possible, if arranged beforehand, to witness the coffin being ‘charged’ - going into the cremator - if desired or required for religious reasons.
At the end of the service, a curtain is usually closed around the coffin, but if this isn’t something you want we can request that it be left open.
After you have had some time to speak to the others in attendance, you will be taken to where you have arranged to be returned, such as the reception venue or home.
The ashes are usually ready for us to collect the next working day, though this is not always the case.
Once we have collected them we will let you know so you can come and collect, or we can deliver them to you at home.
Required paperwork
In order to arrange the service we will need to obtain or complete the following documents to submit to the Crematorium:
Registrar’s Certificate for Burial or Cremation
Known as the ‘Green Form’, this is either given to you at your appointment to register the death or sent to us directly following that appointment.
Application for Cremation (Form 1)
This is your application form, completed either in person with us or via email and electronic signature.
Crematorium Particulars Forms
Not all Crematoria have these, but the local ones do. This form is usually filled in by us but will need a signature from you. It can also be completed either in person with us or via email and electronic signature.
Medical Certificates (Forms 4 & 5)
These forms will be completed by medical professionals, either from a hospital or the local surgery. This is usually collected by us or sent to us directly. These forms will not be required if the Coroner is involved. There is also a fee which we pay on your behalf for these forms, currently £82 each.
Certificate of Coroner
This form will replace the ‘Green Form’ and forms 4 & 5 if the Coroner is involved. This form is usually sent directly to us.
